I haven't been to a doctor in at least 4 years, and i'm moving to another state. Just wondering what sorta information i'll need when getting a new doctor.|||it would be nice if you had some of your medical records. they will want to know about any past surgery's or hospital stays and what meds.
you are taking and what your medical problems are. its been so long since you have been to a doctor they will probably want you to have a physical and blood work to get an idea of your health. when you see a new doctor they will have you fill out new patient forms, just put down what you know and they will go from there.|||You just need your medical records and a good memory :-).

Basically, the doctor will want to know your health history (your past and present health status), and your medical examination findings (what the health-care providers observe when you are examined).

Other information may include lab test results; medications prescribed; referrals ordered to health-care providers; educational materials provided; and what plans there are for further care, including instruction for self-care and return visits.

That will give the clinician a feel for what has happened before to you. As a result, it may often give clues to current disease states. It includes several subsets:

Surgical history

Obstetric history

Medications and medical allergies

Family history

Social history

Habits

The history of vaccination should be included.


Finally, like I hinted, you should be able to talk about most issues concerning your health.
